Acne Scars vs Active Acne — What’s the Difference & How to Treat Them

Many of our clients come in with the same concern: “My acne is gone, but the marks are still there.” Understanding the difference between active acne and acne scars is crucial — because treating them requires completely different approaches.

What Is Active Acne?

Active acne refers to live, inflamed breakouts — including whiteheads, blackheads, papules, pustules, and cysts. These occur when hair follicles become clogged with oil, dead skin cells, and bacteria. Active acne is an ongoing skin condition that needs to be managed and treated at the source.

Common causes: hormonal fluctuations, excess sebum production, bacteria (C. acnes), diet, stress, and using the wrong skincare products.

What Are Acne Scars?

Acne scars are the marks left behind after acne heals. They come in two main types:

  • Post-Inflammatory Hyperpigmentation (PIH): Flat dark or reddish marks left after acne clears. These are not true scars — they are discoloration caused by inflammation and usually fade over time.
  • Atrophic Scars (Pitted Scars): Indented scars caused by loss of collagen during the healing process. These include icepick, boxcar, and rolling scars, and they are more difficult to treat.

How to Treat Active Acne

  • Use gentle, non-comedogenic skincare products
  • Incorporate salicylic acid (BHA) or benzoyl peroxide into your routine
  • Avoid picking or squeezing breakouts — this causes scarring
  • Consider professional treatments like deep cleansing facials, LED therapy, or medical acne facials

How to Treat Acne Scars

  • For PIH (dark marks): Use brightening ingredients like niacinamide, vitamin C, or kojic acid. Always wear SPF to prevent marks from darkening further.
  • For pitted scars: Professional treatments are most effective — microneedling, chemical peels, fractional laser, or collagen-induction therapy can visibly reduce scar depth over a series of sessions.

Important: Always treat active acne first before targeting scars. Trying to fade scars while you still have active breakouts will slow your progress and may worsen inflammation.
